Mental 2 keeps the unmistakable look of the original. We are still locked inside the corridors of a psychiatric hospital, only now the background feels even darker. The walls carry dried messages like “Help Me” and “No Escape”, with peeling paint, rusty doors and switches, and an old sink with medical bottles visible to the side. The opening animation lowers us into the basement in a creaking lift, and when the doors open, the reels look like part of that underground block.
The symbols on the reels are the familiar patients with empty eyes, bandaged heads and metal restraints, alongside anatomy straight from a pathology textbook: a skull, spine, ribs, an embryo in a jar, and a severed foot.
In the Bloodletting bonus, the background shifts to another wing of the hospital. A mode sign lights up above the reels, and the grid fills with flaming frames and scenes from a treatment room. Visually, Mental 2 feels like an even lower level of the same hospital — the place the first game never quite took us to.
🎶 Sound
The sound design completes the descent into madness. The main track is quieter than in the first game — a low hum with uneasy echoes that feels more like waiting for a scream than listening to a typical slot soundtrack. Each spin is accompanied by dry clicks, metallic creaks and sharp sound bursts when features activate, so every xWays or Fire Frame stands out.

Special symbols
To understand how to play Mental 2, it helps to focus on the special symbols and their roles. Almost every one of them is tied to a specific mechanic, turning each spin into a small puzzle.
- Wild. Substitutes for any regular paying symbol and helps form combinations across 108+ ways. It does not pay on its own — its strength lies in connecting patient and anatomy symbols into one line.
- Cognitive Scatter (brain in a jar). Appears only on reels 1, 3 and 5 and triggers free spins. It has no direct payout, but the number of these jars determines which bonus mode you enter.
- Decapitated Scatter (head in a jar). Lands on reels 2 and 4, combining with cognitive scatters to unlock the more advanced Surgery and Experimental modes. In the base game it participates in Mental Transform, turning into one of several strong symbols.

- Enhancer Cells. Closed cells below reels 2, 3 and 4 activate when enough Fire Frames appear. An activated cell reveals an enhancer — a wild, xSplit, xNudge Wild, xWays, or a patient symbol — potentially changing the outcome of the spin.
- Mental Transform. A decapitated scatter landing on reel 2 or 4 can transform into xWays, xSplit, a patient symbol, Dead Patient, or a wild. This transformation often becomes the missing piece needed for a large combination.
- xWays (gold xWays symbol). Turns into several identical symbols and expands the reel vertically, increasing the number of winning ways. All xWays in a spin reveal the same symbol.
- xSplit. On all reels except the one it lands on, it selects a position and splits the symbol into two identical copies. Afterwards the xSplit itself splits into two wilds on its reel, often crowding the grid with symbols.
- xNudge Wild (restrained patient). Always expands to the full reel height by nudging up or down. Each step increases its multiplier by one, and all xNudge multipliers on the screen combine.
- xHole (bloody drain). Activates only if it lands in the centre of reel 3 inside a Fire Frame or Fire Reel. It pulls regular symbols and wilds into the “hole”, then returns them as splits or with larger multipliers before turning into a wild.
- xMental (MENTAL logo in flames). When appearing inside a Fire Frame, it triggers the xMental respin with Fire Frames across the entire grid and boosted Enhancer Cells.
- xGod. An extremely rare symbol needed to reach the maximum win. If it appears on every reel, the game can instantly award the top prize. In God Mode, the entire mechanic revolves around this symbol.
- Spider (black spider symbol). Appears only during spins with xGod. It has no value on its own and simply fills the surrounding cells, highlighting the hunt for the maximum win.
Bonus game
Bonuses in Mental 2 depend on combinations of the two scatter types. Three cognitive brain jars on reels 1, 3 and 5 trigger the basic Bloodletting Spins. If one or two decapitated scatters appear on reels 2 and 4, the bonus upgrades immediately to Surgery or Experimental Spins.
During bonuses, a scatter landing inside a Fire Frame adds +1 spin, while a scatter inside a Fire Reel adds +2.
Bloodletting Spins
- The starting bonus mode uses the same Fire Frames and Fire Reels as the base game. However, all fiery positions remain sticky for the entire bonus. Each chain of splits, and therefore, increases the potential of the grid.
- A decapitated scatter during Bloodletting upgrades the remaining spins to Surgery and adds +1 spin, while two heads immediately upgrade the round to Experimental Spins with +2 spins.
Surgery Spins
- Here, the effects from Bloodletting are expanded with the full Dead Patients mechanic.
- The decapitated scatter becomes sticky and triggers Mental Transform on every spin, while a Dead Multiplier panel appears above the reels.
- When a Dead Patient is revealed, one type of patient is selected together with its maximum multiplier and a counter. These values are then applied to every appearance of that patient symbol on the reels.
- Each time the multiplier is used in a win, the counter decreases, so there is a trade-off between frequent medium payouts and rarer but more powerful hits.
- If a new decapitated scatter lands during Surgery, the remaining spins are upgraded to Experimental, and +1 additional spin is awarded.
Experimental Spins
- The most chaotic free spins mode. Both decapitated scatters become sticky and trigger Mental Transform on every spin, while the Dead Multiplier counter no longer decreases when used.
- Multipliers on patient symbols continue to grow as new Dead Patients appear, and with enough spins, even small combinations can turn into significant payouts.

The downside is obvious: volatility here is extreme, and Mental 2 is often more comfortable to explore in demo mode before risking a full bankroll.
