Mental looks like a scene from a horror movie shot on old film: grainy image, dim light, shabby clinic walls, a dirty metal table, X-ray images, and patients glancing ominously. The rusty-brown palette makes it feel as if the screen smells of antiseptic and something sinister.
Symbols are organically integrated into the overall aesthetic: phalanges of fingers, lungs, hearts, eyes. Patients are minimally animated, but enough to cause discomfort. And that’s a compliment.
🎶 Sound
The sound in Mental slot isn’t just background — it’s part of the horror. Instead of music, there’s the anxious hum of ventilation, creaking doors, muffled knocks, and short, sharp phrases from patients. It’s like a bad dream where you’re not sure if you’ve woken up.
During spins, you hear mechanical clicks and taps, as if someone on the other side of the wall is counting down the free spins. On a big win — no fireworks, just metallic clangs, an ominous hum, and distorted screams.

Special Symbols
Mental has no traditional paylines or wilds but features unique symbols and signature mechanics from Nolimit City:
- Scatter (scorpion): three venomous tails land and trigger Autopsy Free Spins. An additional spider upgrades to “Lobotomy”, and two spiders lead to Mental Free Spins, where things get truly crazy.
- Spider: jumps onto reels 2 or 4, triggers Mental Transform, and immediately transforms — turning into a wild, xWays, xSplit, or even Dead Patient. In bonuses, this eight-legged creature also “sticks”, like tape to gloves.
- xWays: one symbol turns into multiple identical ones, expanding the grid and increasing matching chances.
- xSplit: splits symbols in half, doubling their number and boosting multipliers.
- xNudge Wild: a multiplier symbol that “nudges” across the entire reel to enhance wins.
- Dead Patient: an expensive, silent clinic resident; in regular spins pays well, and in free spins collects multipliers that can resurrect even a critical balance.
All these effects work in tandem: xWays expands the grid, xSplit boosts multipliers, and the spider triggers it all — eventually causing win amounts to skyrocket. Or not — luck decides.
Bonus Game
Mental has three free spin levels — the further you go, the tougher it gets. All activate via Scatter symbols (lamps), each granting more multipliers and chances for x66,666.
Autopsy Freespins
8 free spins awarded. Enhancer Cells under reels 2 and 4 work here. They can trigger xWays, xSplit, the spider, and other effects. Good combos are possible even at this level, but without the Dead Multiplier.
Lobotomy Freespins
Requires 4 lamps or upgrade from Autopsy. Again, 8 spins given, but with Dead Multiplier: a random multiplier (up to x9999) applied to wins involving two specific patients. The multiplier can stack.
Mental Freespins

Mental has several RTP versions depending on the casino and game mode. In its basic configuration, the slot offers:

Our Verdict
In short, Mental’s rules complexity is off the charts — the complete opposite of simple classic slots like Big Bass Bonanza. Sometimes it feels more like a puzzle than a slot.
The game leaves strange feelings: the atmosphere wraps you up, the mood is set, but you can’t grasp the mechanics. At first, you try to understand, then just start counting multipliers and bonus returns and stop overthinking. On the other hand, fans of digging into slots and figuring out what triggered what will probably love it.
In demo mode, we managed to trigger free spins after about 100 rounds. Bonus buys returned about 1-2 times out of 5, but the excess could be substantial. We didn’t stay in profit after 500 spins but didn’t go heavily negative either. So be prepared for very high volatility and a long wait for a big win.
